Government issued valid photo ID

Accepted IDs are Drivers License, MVA ID, Passport, Armed Services card, and Resident Alien card. Name and Date of birth MUST match with your insurance.

Your referral

Please bring your referral from your primary care physician if so required by your insurance.

Current Medications

A list of current prescriptions and/or any over-the-counter medications you are currently taking, including the dosage and frequency. Please include the name and address of your pharmacy for prescriptions.

Medical Records

Please provide the medical records pertaining to your visit.

Brain and Spine Tests

If there are any recent blood work, X-Rays, MRIs, or CT scans of Brain and Spine conducted please bring those copies.

Insurance and Payment Information

Neurological Medicine, P.A., is a provider for Medicare and most major insurance plans. We provide insurance billing. Anything not covered by your insurance company will be your responsibility.

 

We request payment at the time of your appointment for services that are not covered by your insurance. Your insurance company may also require that you pay a co-pay at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.
